Download this form to cancel any of your SSLI Coverage. In order to complete the cancellation process, you must fill out the cancellation form and whichever allotment form you need. If you are cancelling SSLI in its entirety (self and dependent or spouse coverage), you will use the allotment cancellation form. If you are decreasing coverage because you have dependent that are no longer elibible but wish to keep your Service Member coverage, you will fill out the allotment change form.
